This is your chance to be part of an incredible movement and show your support for the University of Miami Miller School of Medicine’s Progression Across Transition in Healthcare (PATH) clinic in the Department of Medicine.

 

Our new PATH clinic supports adults with intellectual and developmental disabilities and chronic health conditions who do not have a medical provider in our community. Our mission is to provide an accessible medical home to close the gap in quality primary care for adults with ID/DD. Our vision is to create the standard for innovative and inclusive care to enrich the quality of life of adults with ID/DD.

Our clinic has been operational for four years and the clinic is growing rapidly. We currently serve over 80 patients. 

Today is Give Miami Day, and you have the power to make a difference. Your generosity will support us in making the clinic more accessible for adults with disabilities, like purchasing an accessible scale for wheelchair users and funding care coordination advocates to help patients and families find needed services that can help them thrive in adulthood.
